I give my permission for my teenager(s) to attend the “Summer Youth Trip” with the Bible Baptist Youth Group. I understand transportation will be via a Chartered Bus Service and BBC bus driven by licensed CDL drivers. I understand that activities will include overnights in a hotel, Bible centered messages, and other “teen-type” activities outlined on the BBC handouts (or that might be expected on a “summer trip” for teenagers). I agree not to hold Bible Baptist Church, Camp Cobeac, and/or their representatives liable in the event of an accident or injury to my teenager(s).
Additionally, I give my permission to any BIBLE BAPTIST ADULT LEADER: (Josh or Faith Miller, Jerry or Teresa Stephens, Wally or Amanda Miller, Alex or Emily Fetherland) to consent for emergency treatment at any local hospital Emergency Room for my child
