Bible Baptist Church Homeschool Co-Op (“Co-Op”) is a ministry of Bible Baptist Church (“BBC”) that provides recreational activities (listed below) as part of our ministry. All activities include some level of inherent risk of injury that can never be eliminated regardless of how much emphasis is placed on safety. This Permission Form covers all activities associated with the Co-Op. The following list includes many, but not all activities the Co-Op participates in:
Art projects that can include cutting with scissors and/or use of glue, outdoor games, basketball, dodgeball, wiffle ball, roller skating, ping pong, broomball, water balloon toss, tug of war, science experiments, gardening, nature activities, cooking/baking lessons, gym games, and other similar activities. A parent/guardian is required to be on-site with their children during all activities. If you do not want your child to participate in a particular activity, please inform the teacher that day. Additionally, use the space below to indicate any activities you do not want your child to participate in.

I take full responsibility for ensuring that my child refrains from participating in these activities and will personally monitor their involvement.

I, the undersigned, acknowledge and understand that the Co-Op is providing food and beverages during each meeting. I acknowledge that it is my responsibility to ensure that the food consumed by my child is safe and appropriate given any allergies, intolerances, or dietary restrictions they may have. I acknowledge that neither BBC nor the Co-Op can guarantee the avoidance of allergens or other food ingredients that may be harmful to my child, and that cross-contamination may occur during preparation or service.

BBC is hereby authorized to use photos of my child in publications, its website, and/or on social media for purposes of promoting the co-op and for public relations for BBC.

I understand and hereby agree to assume all risks which may be encountered on said activity, including activities preliminary and subsequent thereto. I do hereby agree to hold Bible Baptist Church and its agents and employees harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to my child or property, even injury resulting in death, which my child now has or which may arise in the future in connection with the activity or participation in any other associated activities. I expressly agree that this release, waiver, and indemnity agreement is intended to be broad and inclusive as permitted by the law of the State of Ohio, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. This release contains the entire agreement between the parties hereto, and the terms of this release are contractual and not a mere recital. I further state that I have carefully read the foregoing release and know the contents thereof, and I sign this release as my own free act. This is a legally binding agreement which I have read and understand.
