2024 Annual Youth Group Permission Slip








Bible Baptist Youth Group is a ministry of Bible Baptist Church (“BBC”) that provides recreational activities (listed below) as part of our ministry. All activities include some level of inherent risk of injury that can never be eliminated regardless of how much emphasis is placed on safety. This Permission Form covers all activities of durations up to and including overnight (one-night) outings. A separate permission slip will be required for the Youth Summer Trip.

The following list includes many, but not all activities the youth group participates in: Air Hockey, Airsoft/Paintball, Amusement Parks, Archery tag, Basketball, Batting Cages, Bonfire, Bounce Houses, Bowling, Canoeing, Capture the Flag, Cooking/Baking, Crazy Olympics, Cyclones Hockey, Dodgeball, Door-to-door visits, Driving Range, Egg toss, Fishing, Flag football, Go carts, Golf, Hay rides, Hiking, Horseback riding, Ice skating, Laser Tag, Lock In, Mini Golf, Nine square, ping pong, Progressive dinner, Racquetball, Reds games, Rock climbing, Roller skating, Scavenger hunt, Snow tubing, Soccer, Softball/baseball, Spikeball, Swimming, Tent Camping, Track and field games, Trade up, Trampoline park, Tug of war, Ultimate frisbee, Volleyball, Water balloon toss, Whirley ball, Wrestling/Horseplay, Youth rally, Zoo

I Approve
Transportation will be provided for all Youth Activities once your teen arrives at Church. In most cases, transportation will be provided via bus using operators with a valid commercial driver’s license. In some cases (e.g. teen follow-up visitation) the following may occur:
- Adult youth workers may transport teens in their personal vehicles and be the only adult in the vehicle
- Approved Teens (age 18 or 19) may transport other teens in their vehicles with no Youth Leader present in the vehicle

Additional Transportation
 Transportation will be provided for any services at Bible Baptist Church. In most cases, transportation will be provided via the personal vehicles of child-safety approved adult workers.

I Approve
The following will be adhered to during any overnight activity:

- Teens and youth workers will always be separated by gender for sleeping arrangements
- No one will be permitted in a room reserved for the other gender
- An individual adult worker will never be alone in a room with only one teen
- One adult worker may be sleeping in a room with two or more teens
- Occasionally two or more teens may be sleeping in a room with no adult worker

I do not wish for my child to participate in the following and will personally remind the Youth Workers of such:



I understand and hereby agree to assume all of the risks which may be encountered on said activity, including activities preliminary and subsequent thereto. I do hereby agree to hold Bible Baptist Church and its agents and employees harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to my child or property, even injury resulting in death, which my child now has or which may arise in the future in connection with the activity or participation in any other associated activities. I expressly agree that this release, waiver, and indemnity agreement is intended to be broad and inclusive as permitted by the law of the State of Ohio, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. This release contains the entire agreement between the parties hereto, and the terms of this release are contractual and not a mere recital. BBC is hereby authorized to use photos of my child in publications, newspapers, newsletters, its website, and/or on social media for purposes of public relations for BBC. I further state that I have carefully read the foregoing release and know the contents thereof, and I sign this release as my own free act. This is a legally binding agreement which I have read and understand.
